Intermittent HD assumes high-flux hemodialysis. CRRT assumes CVVHD with ultrafiltration rate 2L/h and residual native GFR.

General Considerations

- 372 mg of isavuconazonium = 200 mg of isavuconazole.

UCSF: Restricted to ID or Antimicrobial Stewardship except:​

IV formulation: Inability to tolerate or absorb enteral medications AND one of the following:

PO capsule: One of the following:

1) Documented or suspected serious fungal infection in a patient with prolonged QTc interval​

2) Prophylaxis against fungal infections in patients on the HBC service or lung transplant service with prolonged QTc interval or concurrent QTc prolonging medications.
