Dosing: Adult Antimicrobial Dosing, Non-dialysis

Indication CrCl ≥ 30  mL/min  CrCl < 30 mL/min
Prophylaxis in neutropenic fever in patients who are intolerant of fluoroquinolones AND no history of ESBL/CRE organisms  200 mg po q 12h 200 mg po q 24h

Dosing: Antimicrobial Dosing in Intermittent & Continuous Hemodialysis

Continuous Hemodialysis
Prophylaxis in neutropenic fever in patients who are intolerant of fluoroquinolones AND no history of ESBL/CRE organisms  200 mg x 1 then 200 mg po daily (on days of HD, administer after HD session)  200 mg po q12h
