71-80 kg 1400 mg PO q24h 20-25 mg/kg PO three times weekly
81-90 kg 1600 mg PO q24h 20-25 mg/kg PO three times weekly

Dialysis Notes

Intermittent HD assumes high-flux hemodialysis. CRRT assumes CVVHD with ultrafiltration rate 2L/h and residual native GFR < 10 mL/min.  For detailed view of dialysis dosing and evidence, see Dosing in Hemodialysis document.

Dose based on ideal body weight. Dosing in obesity (BMI > 30): use lean body weight (see online calculator).

Drug is available in 400mg and 100mg tablets.
