Ethambutol

Dosing: Adult Antimicrobial Dosing, Non-dialysis

This dosing aligns with current practices at the SFDPH TB Clinic (as of 5/2025) for treatment of tuberculosis. For patients in other counties, contact the respective TB clinics for dosing recommendations. 

Indication CrCl > 30 mL/min < 30 mL/min
Tuberculosis

20 - 24 mg/kg PO daily

Max starting dose = 1600 mg. For patients > 80 kg, contact ID pharmacy for dosing advice. 

20 - 24 mg/kg PO three times weekly

Non-tuberculous mycobacteria (MAC)

15 mg/kg PO daily 15 mg/kg PO three times weekly

Therapeutic drug monitoring may be utilized under the guidance of SF TB Clinic, ID, and/or ID/ASP Pharmacy. Higher dosing may be required.

Indication

Intermittent Hemodialysis

Continuous Hemodialysis
Tuberculosis 20 - 24 mg/kg PO x 1 now and post-HD 15-20 mg/kg PO q24h
Non-tuberculous mycobacteria (MAC) 15 mg/kg PO x 1 now and post-HD 15 mg/kg PO q24h

Therapeutic drug monitoring may be utilized under the guidance of SF TB Clinic, ID, and/or ID/ASP Pharmacy. Higher dosing may be required.

Dialysis Notes

Drug is available in 400mg and 100mg tablets.
